Service Application

Applying for service is easy!

Once you know what services you would like to subscribe too, just click on this link below and fill out the application and return to Huxley Communications by email, in person, or by postal service. If you are unable to view this online form, a paper version is available by clicking HERE. Please remember a $25.00 cooperative membership fee is requested for all phone and internet customers. For any new customer that has not provided a positive letter of credit, we also require a $75.00 refundable service deposit. The application, membership fee and service deposit, if applicable, are all required before service can be installed.

  Please save this completed application to your computer and then email the file as an attachment to huxtel@huxcomm.net. It will be saved as an Adobe Acrobat PDF form.
• RETURN TO OFFICE OR MAIL
